I agree with Mike!
The Fork will work - it is a 2 of 2 fork. In your loop, when the control comes for the decision, if it is in a 2 of 2 fork, then the system will send the decision in one branch and WAIT till it is approved/rejected AND in the other branch, it will send the email and then wait for the first branch to complete. Instead of the email step, you can use a background step with a method containing your own code (which you are using the program exit).
Extended Notifications gives you the links for approval and rejection (however, the user has to login to SAP to do the actual execution.....AND that is recommended and SECURE .... and I am NOT a supporter of direct approvals from emails!! It's insecure.)
AND If you are using specific Agent Assignment at the task level or using Excluded Agents, you have to take care of those as well while sending the email (as pointed by Stephane).
