Dear Godfrey,
If just one copy in the output device set and your print page smartform has just one also, I believe that your problem has something related with the Device Type definition in your Output Device.
In this case I recommend you to ask some assistance from Basis Team showing them all of your analysis and also the Spool request with one copy and one page.
Maybe your problem is happening with the interface between SAP Device setup and the printer(hardware).
